I am writing to you today to request that you supply me with information contained within yourfiles. The information I am requesting is the last known address for my Birth Brother__________________. I am requesting this information for two reasons. One is that you have stated that this address is no longer good so therefore it is not IDENTIFYING INFORMATION. The second is that I need to find my Birth Brother in order to make him aware of life threating medical conditions to which he is pre disposed.

I am requesting that you handle this request for information immediately and without delay as it is a medical situation. I also request that if for any reason you deny my request for information, you do so in writing to me at the above address in a timely fashion and that with that denial you include your legal reasons and supporting laws which justify your denial. If you do issue a denial letter it must include your name and title and authority to make that decision. I will need that information for my attorney.
