If the adoptee was adopted by a private agency such as Waverly childrens home,
Boys and girls aid society you should contact the appropriate agency directly to register for the services the voluntary registry can provide.
Non-id info can be requested by bio parents of adoptee ages 18+, adoptive parents or guardians of the adoptee.
If the adoptee is deceased the adoptees spouse is eligible to register.
To receive non-id you must Request the form entitled "Request for Non-identifying Information" and send $45.00 in with the form.
A 1997 Law allows a putative father to register for Idenifying information ONLY.
Identifying information will be given only when adoptee AND one of the parties below have registered with the registry for this disclosure.

return "Affidavit for Identifying Information form completed and notarized with a copy of your birth certificate not hospital certificate and a check or money order payable to Services to Children and Families in the amount of $25.00.
